Case Study: Assessment, Crisis Intervention & Care Management
Clayton B. was 87 years old
and living alone in Phoenix without outside assistance. When he
began to show signs of serious deterioration, his son, Paul B.,
who lives in Ohio, became concerned. He turned to A.G.E. Consultants
for help for his father.
We met with Clayton on a Friday
to assess his situation. Clayton's needs were twofold: immediate
assistance in his present situation and long-term help with relocation
to an appropriate assisted living facility. We arranged for medical
appointments, meals, housecleaning and transportation to begin the
following Monday while we began looking into an appropriate living
facility. The next day, however, we received another emergency phone
call: Clayton had fallen and was in the hospital. Could we please
intervene?
We visited with Clayton, coordinated his care
with the physicians and hospital staff, and arranged for his discharge
into a care facility, where we advocated for the treatment he needed.
When Clayton's insurance denied him further treatment and placed
him on private pay care, we appealed, and won the decision. When
Clayton was again rushed to the hospital and diagnosed as terminal,
we coordinated his discharge into a Hospice facility, and provided
personal support and coordination of all of Clayton's end-of-life
needs. We alerted Paul when to come out to visit Clayton so they
could have some quality time together. We remained with Clayton
to support him at the time of his death, and carried out all of
his burial wishes in coordination with Paul.
